Care after a hospital stay in Leeds

The first few weeks back from hospital are when people are most at risk of a fall or a return admission. Having someone at home to help with meals, medication and moving about can make all the difference to recovery.

Live-in care after hospital can be for a few weeks or longer, depending on how quickly you get back on your feet.

Could care after hospital help?

It's often a good fit when:

  • You're recovering from surgery - a new hip or knee, for instance
  • An illness or infection has left you weak
  • You were admitted after a fall
  • Hospital staff are asking what support you'll have at home

What your carer helps with

  • Getting home and settled on the day of discharge
  • New medication, taken correctly
  • Help on the stairs, in the bathroom and getting in and out of bed
  • Following the exercises given by physiotherapists
  • Nourishing meals and plenty of fluids
  • Getting to follow-up appointments

What a day can look like

  1. Discharge day

The carer meets you at home, checks the house is ready and the prescriptions are collected.

  2. First week

Plenty of rest, help with every transfer, and meals made for you.

  3. Weeks two to four

More walking, more independence, exercises done each day.

  4. Review

We check in on progress and agree whether to reduce, continue or end support.

Every day is planned around the person - this is only an example.

Leaving hospital in Leeds

The Leeds Teaching Hospitals discharge teams at the LGI, St James's and Chapel Allerton will want to know you have support at home. Telling the ward early that a live-in carer is arranged can help plan a smooth discharge. Leeds City Council may also offer a short period of free reablement support for some people.

Short notice is fine

Discharge dates can change at the last minute. Let us know as soon as you have an idea of timing and we'll keep in touch with you and adjust if it moves.

Questions about care after hospital

Yes. We can speak with your family, and with ward staff if you agree, to understand your needs before you're discharged.

That's fine. Short-term live-in care after hospital is common, and we'll agree the length at the start.
