Before discharge

At the LGI, St James's and the other Leeds hospitals, discharge planning should begin soon after admission. Ask the ward team early what support they think will be needed and when discharge is likely.

  • Ask whether physiotherapy or occupational therapy will continue at home
  • Ask about equipment - a commode, frame, rails or a hospital bed
  • Ask whether short-term reablement support from the council is being arranged
  • Tell the ward if you're arranging private or live-in care

On the day

Discharge days can run late. It helps to have someone ready at home.

  • Heating on and the bed made up, downstairs if needed
  • Fresh food and drinks in the house
  • Prescriptions collected - check the discharge letter
  • A clear path through the house, with rugs and trip hazards moved

The first few weeks

Falls, infections and not drinking enough are the big risks in the first weeks home. Keep an eye on eating and drinking, follow the exercises the therapists have given, and keep follow-up appointments.

If you're worried someone won't cope, short-term live-in care can bridge the gap until they're stronger.
